The existing sample.pdf renders in PDFKit, so it cannot exercise a failed-PDF fallback. Add an explicitly malformed PDF with a missing page tree and a Markdown page that embeds AVIF, ICO, SVG, two-frame GIF, TIFF, and a missing image with descriptive alt text. Both fixtures are generated and indexed with the rest of the corpus.

Validation: python3 scripts/generate.py produced 228 fixtures and python3 scripts/verify.py passed. macOS PDFKit returned no document for malformed.pdf.
